You are invited to participate in a study on breaches of electronic
resources licenses in academic libraries.  The goal of this project,
conducted by Kristen Kruse from the University of Manitoba, is to
gather information on the experiences of academic libraries with
license breaches, thereby providing an idea of how the frequency,
severity, and policies of breaches compare between libraries.

If you are familiar with breaches of licenses for electronic resources
at an academic library, you are encouraged to complete a short survey
(link below). This survey consists of 10 questions and should take
approximately 15 minutes to complete.

Participation is voluntary and you are free to withdraw from the study
at any time, and/or refrain from answering any questions you prefer to
omit, without prejudice or consequence.

The data collected is of a general nature; no detail of specifics,
including the products or parties involved, will be covered.  There
are no perceived risks with this survey.  No personal information will
be collected and any identifying comments will be rendered anonymous
prior to analysis.

You may not receive any direct benefit from participating in this
study, but I plan to share the results at the Electronic Resources &
Libraries conference in March 2013, and hope to publish a paper with
the results and analysis.

This study has been approved by the Joint-Faculty Research Ethics
Board at the University of Manitoba. If you have questions or concerns
