Just a quick note about the context of this report by Andrea Kwan. It
was based on a White paper she prepared for the Canadian Association
of University Presses in 2011:

It was subsequently expanded and submitted as a Master's thesis at
Simon Fraser University, also in 2011. Someone decided to upload the
thesis recently, and it has been picked up in social media, and this
may have given the impression that this is a new study. This explains
the lack of reference to many of the recent developments.
