Dear Colleagues,

You are invited to participate in an online survey about perpetual
access to electronic serials.  The survey seeks to ascertain the
extent to which perpetual access is being pursued and provided for
electronic serial titles that have been canceled, have ceased, or have
transferred to different publishers.  If you work at a library that
has had the opportunity to test perpetual access provisions - whether
successful or not - your participation would be very much appreciated.

Results of this survey may be included in a future publication and/or
presentation.  All answers will be kept confidential.   The survey is
short and should take no more than 10 minutes to complete.
